Free Advance Directive Notarization Event

Take Action on Your Health Care Wishes. FirstHealth Hospice & Palliative Care is offering free events to help community members complete and notarize their Health Care Power of Attorney and Living Will documents. These important documents allow you to clearly communicate your health care wishes and ensure your loved ones understand your preferences during medical emergencies or end-of-life care. Community members can attend one of two events where notaries and witnesses will be available at no cost to finalize your documents.

How to Prepare

Participants should:

1. Call Maggie Bonecutter at (910) 715-3408 to schedule a 10-minute appointment.

2. Visit FirstHealth.org/advancedirectives to download and print the Health Care Power of Attorney and Living Will forms.

3. Discuss your health care wishes with loved ones.

4. Complete all sections of the forms except the “County of” line and your signature.

5. Bring the documents and a photo ID to your appointment.

Notarization is limited to one Health Care Power of Attorney and one Living Will per person.
